Monday – Friday, 8:00 AM – 4:30 PM

Sapphire Recruitment is seeking a Care Coordinator to provide administrative and technical support for a healthcare team in Syracuse, NY. This role is ideal for a detail-oriented professional with strong customer service and organizational skills.

Key Responsibilities:

  • Conduct outbound member calls to verify information and provide assistance.
  • Assist case management with scheduling and coordination of patient services.
  • Support Care Managers by tracking member contacts, visits, and authorizations.
  • Manage reports within electronic health records (EHR) and maintain accurate documentation.
  • Answer and triage phone calls, addressing inquiries or escalating as needed.
  • Perform general administrative tasks, including filing, copying, faxing, and data entry.
  • Collaborate with team members to improve processes and enhance member support.

Qualifications:

  • High school diploma required; Associate’s degree preferred.
  • 2+ years of experience in healthcare administration or as a Medical Assistant.
  • Strong computer proficiency, including Microsoft Office and EHR systems.
  • Excellent communication, problem-solving, and organizational skills.
  • Ability to work independently and adapt to shifting priorities.
  • Attention to confidentiality when handling sensitive information
  • Physical Requirements: Extended computer use, occasional lifting (up to 40 lbs.), and frequent sitting and standing
